This short survey, being given statewide right now, will only take you about five minutes to complete, but the information you provide will be extremely valuable in helping us pinpoint our strengths and identify where we can do better. Your responses, as well as those from teachers and students in 6th – 8th grade, will show us how we fare in the five areas that have the most impact on student outcomes:
The survey is anonymous; no parents, students or teachers will be identified in the school reports, which only present summary information. You can view the privacy policy online.
Follow this link to take the survey at your convenience – but please log in before March 31, when the survey closes.
The Illinois State Board of Education provides answers to frequently asked questions about 5Essentials. If you have other questions about the survey, please contact 5Essentials Customer Support at 1-866-440-1874 or 5essentials@uchicago.edu.
